Check List No. 2
For Responding Company

  1. Select Supervisor or Supervisors who will be in charge.
  2. Instruct Supervisor in charge of the duties expected of him.
  3. Decide on mode of transportation, based on weather, distance, time of day and available transportation.
  4. Set up transportation details and give to Supervisor in charge. If distance is greater than 200 miles, work forces should be dispatched by public transportation.
  5. If trucks should be required:
    1. Select and assign drivers.
    2. Select trucks.
    3. Have trucks serviced.
    4. Load any special tools and material if requested.
    5. Load rubber coats and boots.
    6. Assign Supervisor, if necessary, to accompany fleet.
    7. Arrange meeting place for trucks dispatched from different locations.
    8. Provide for expenses from home base to destination.
    9. Arrange departure time and notify Requesting Company.
    10. Advise Requesting Company as to:
      1. Departure time.
      2. Approximate arrival time at designated location.
      3. Name of Supervisor in charge, number of men and vehicles.
  6. Provide Supervisor with:
    1. Name and address of Requesting Company.
    2. Name, address and phone number of person he is to report to in Requesting Company.
    3. Conditions of emergency, i.e., rain, snow, wind, lightning, flood, etc.
    4. Estimated duration of emergency. It is the responsibility of the requesting utility to arrange housing facilities, if needed.
    5. Equipment needed.
    6. Tools required.
    7. Weather - present and forecast.
  7. Provide Supervisor in charge with check list of personal items required by employees, such as change of work clothes, personal toilet articles, shaving equipment, tool bad (which includes gloves, goggles, coveralls, company uniforms, work shoes, hard hats, etc.)
  8. Provide Supervisor and men with money and instructions to handle required expenses.
  9. Notify top management, through proper channels, of assistance being provided and to whom.
  10. Dispatch Supervisors, personnel and Equipment to destination as suggested by Requesting Company.
  11. Provide Foreman and/or Supervisor with time slips, report forms and other required stationery supplies.
  12. Provide crew members with I.D. Cards and, if required, authorized CIVIL DEFENSE PASSES.
  13. Obtain any special insurance coverage desired for duration of the emergency.
