Information Supplied by Requesting Company to Responding Company

  1. Name and address of Requesting Company.
  2. Name and title of person calling.
  3. Telephone number where requester can be reached.
  4. When the help is wanted.
  5. Where the help is wanted.
  6. Where the help is to report.
  7. The name and title of person to report to.
  8. Number and types of crews and Customer Service fieldmen requested.
  9. Kind of work to be performed.
  10. Type of emergency: rain, snow, sleet, wind, earthquake, fire, lightning, flood, other.
  11. Estimated duration of the emergency and extent of outage.
  12. Work to be done on mains, services, meter set assemblies, restoring service.
  13. Equipment needed:
    1. Construction crew trucks
    2. Passenger cars
    3. Other trucks (Customer Service trucks, dump, regulator, pressure control)
    4. Welders
    5. Compressors, backhoes, skiploaders
  14. Materials needed.
  15. Expense money required to provide for trip from home quarters to destination.
  16. Weather - Present and forecast.
  17. Tools required, including pneumatic tools, pressure control equipment, pipe locators, leak detectors, pumps, lighting equipment, etc.
  18. Travel conditions for transportation facilities - airport, highways, railways.
  19. Suggested mode of transportation for distances greater than 200 miles. Trucks should be dispatched separate from the work forces. Mechanic to accompany if deemed necessary.
  20. Suggested highway routes to travel.
  21. The name and title of person meeting Responding Company employees at point of arrival.
  22. Coordinate difference in time, as to time zones, standard time and daylight savings time.
  23. Housing and food facilities available. It is the responsibility of the requesting utility to arrange for housing facilities, if needed.
